What is a Support Coordination Agency?

Individuals in New Jersey with a disability ages 21+ are eligible to receive funding from the Division of Developmental Disabilities (DDD). When an individual is approved to receive funding from DDD they are assigned a Support Coordinator.

Role of a Support Coordinator

  • To connect individuals and families with resources and service providers

  • To educate them about the DDD funding and services

  • To regularly monitor the implementation of the services to ensure they are effective and meeting the individual's needs

  • To help the individuals and families utilize their budget in the most productive way.
