What is a Support Coordination Agency?
Individuals in New Jersey with a disability ages 21+ are eligible to receive funding from the Division of Developmental Disabilities (DDD). When an individual is approved to receive funding from DDD they are assigned a Support Coordinator.

Role of a Support Coordinator

To connect individuals and families with resources and service providers

To educate them about the DDD funding and services

To regularly monitor the implementation of the services to ensure they are effective and meeting the individual's needs

To help the individuals and families utilize their budget in the most productive way.
